The Adopter agrees to adopt the following animal (hereinafter called "Animal") from One Dog At A Time Rescue (ODTR):

As Adopter, I agree to the following:  (please read, understand and initial)
To allow a representative of One Dog At A Time Rescue to visit my premises to insure the terms of this agreement have been kept; _____
The Animal will not be kept strictly outdoors on a chain or other tie-out device, and not allowed to run unattended in any unrestricted area outside;  _______
The Animal will be provided with adequate fresh food and water, clean, dry shelter when outside, and daily exercise; _______
To provide a safe collar with rabies and I.D. tags to be worn at all times; ______
To obey all applicable laws governing control and custody of animals;  ________
To provide all medical care and treatment needed by the Animal including but not limited to: yearly medical checkups and vaccines, and preventative heartworm medication;  _____
To adopt the Animal only as a personal pet/companion and not as a gift, working animal, or guard dog; ______
The Animal is not be be sold or given away for any reason. If it becomes necessary to find the Animal another home, One Dog At A Time Rescue will be contacted and the Animal shall be returned to them or arrangements will be made to have One Dog At A Time Rescue approve the new home I would like to place the animal in and a new Adoption Application and Adoption Contract will be required for the potential new owner;  _____
If the Animal has not been spayed/neutered at the time of this adoption, a $_________ deposit shall be required to be paid to One Dog At A Time Rescue at the time of the adoption.  Upon written proof (which WILL be verified) from your veterinarian, this deposit will be refunded to you within fifteen (15) days of the verified proof of the spay/neuter.  __________
If the Animal has not been spayed/neutered at the time of the adoption, I agree to have the surgery done by _____________. In the event I do not have the Animal altered by the date agreed upon then ownership of the Animal shall revert to One Dog At A Time Rescue upon demand.   _____________

One Dog At A Time Rescue assumes no responsibility nor shall they be held liable for any damages caused by the adopted Animal to property, person or other pets. If any medical problems should occur after adoption, One Dog At A Time Rescue is not liable for any medical bills.   _______________
As Adopter, I agree to accept full responsibility for the care of this animal releasing the previous owner as well as One Dog At A Time Rescue and its representatives of any liability from this date forward. ______________
I agree to hold harmless One Dog At A Time Rescue and/or any representative of One Dog At A Time Rescue, for any injury sustained to myself, family members, pet and/or friends as a result of having adopted this animal.  _________
I agree to abide by the adoption conditions and realize that I am committed to this animal for his/her life. I have read and fully understand that One Dog At A Time Rescue may repossess this animal at any time if the adoption conditions are violated or the animal is mistreated. _____________________
Attorney's Fees and Court costs: Adopter agrees to pay reasonable attorney's fees and court costs in the event this matter is forwarded to an attorney for enforcement.  ________
The Adopter represents that he/she has never been subject to legal action for cruelty to or neglect of animals. The Adopter further represents that he/she has never owned an animal which has been confiscated by any animal control or humane organization for violations of state or local animal control regulations or animal adoption agreements.
